New Patient Registration
Thank-you for chosing the Falkland Surgery. Information about the surgery and the surgery team are on the website including how to request appointments and prescriptions.
If you would like to register with the practice please complete the following form.
Please also have your NHS number, which can be obtained from your current practice or on a medical/clinic letter.
Please ensure that you are a resident in our catchment area and living at the address provided to us before registering as a new patient with the practice.

Register for Online Services
To register for our online services you will need to complete the Online Registration form and then visit the practice, bringing with you two forms of identification. One of these items should include your photograph.
We will then issue you a username and password.
Once you are registered you will be able to use the service to:
- Order your repeat prescriptions
- Make an appointment
- Cancel an appointment
- View your medical record
Your Named GP
From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
Our GPs run personal patient lists and whenever possible prefer patients to see their own GP to ensure continuity of care. If for any reason a patient’s GP is not available or a patient would prefer to see a female or male GP, our reception team will always book the patient in with another GP.
If you are unsure who your named GP is, contact our reception team who will be able to advise you.
Temporary Services
If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.
To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.
